The Ayushman Bharat Golden Card allows beneficiaries under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY) to access cashless treatments at empanelled hospitals across India. You can apply online or offline by following the below steps.

• Online Application Process

Here’s the step-by-step guide to apply for the card online –

Step 1:

Visit the official website of PM-JAY: https://pmjay.gov.in/. Click on `Am I Eligible’.

Step 2:

Login, select the Beneficiary option, and enter your mobile number and the Captcha Code to receive an OTP.

Step 3:

Enter the OTP. Provide your details, such as name, mobile number, ration card number, state, etc., to check your eligibility.

Step 4:

Locate the nearest Common Service Centre (CSC) by clicking on `Find CSC’ on the website.

Step 5:

Visit the CSC along with the required documents for verification. You have to undergo biometric verification using your Aadhaar Card for authentication.

Step 6:

Upon successful verification and approval, your Ayushman Bharat Golden Card will be generated. You can download it from the website. If you need a printed copy, you can get it at the CSC by paying a nominal fee.

• Offline Application Process

The process to apply for the card offline is as below –

Step 1:

Go to the nearest CSC along with the required documents.

Step 2:

The CSC officer will verify your eligibility using your Aadhaar or mobile number.

Step 3:

Provide the necessary documents. Once the application is verified and approved, the Ayushman Bharat Golden Card will be issued.

Documents Required to Apply for Ayushman Bharat Golden Card

The list of documents required for the Ayushman Bharat Golden Card includes –

• Identity Proof:

The Aadhaar Card is mandatory

• Address Proof:

Ration card/utility bill (water or electricity)/voter card/driving license

• Proof of Eligibility:

Income certificate or caste certificate

• Family Details:

Aadhaar Number, age, or family photographs

• Additional Details:

Mobile number, bank account number, or email ID

Who is Eligible?

The PM-JAY is meant to provide free healthcare services for the economically weaker sections of society. The applicant must meet the below eligibility criteria to become a beneficiary of the scheme –

1. Rural Households

• No adult members between 16 and 59 years of age

• Households with a single room with kuccha walls and roof

• No adult male members between 16 and 59 years

• Households with differently-abled members and no able-bodied adult members

• Landless households earning a livelihood through manual labour

• Scheduled Caste/Scheduled Tribes

2. Urban Households

• Ragpickers, beggars, domestic workers

• Street vendors, cobblers, hawkers, and others in informal occupations

• Construction workers, plumbers, and masons

• Artisans, tailors, and other similar low-income workers

• Security guards and cleaners

3. Other PM-JAY beneficiaries include homeless individuals, families depending on charity or alms, and families living in tribal areas, slums, or makeshift arrangements.
