g. Form 38A – Inspection report of the bike.

i. Form 60 – Only if there is no PAN card.

a. Form 25 duly filled out,
b. Original registration certificate of the bike,
c. PUC or pollution under control certificate,
d. PAN card of the applicant,
e. Proof of payment of road tax,
f. Fitness certificate for the bike,

h. Proof of Engine number and chassis number of the bike.
